Description: This presentation will compare PACT implementation in rural and urban clinics across a national sample using the PACT Implementation Progress Index (PI2), a composite measure reflecting 8 core Patient-Centered Medical Home domains. A comparative case study of five Primary Care CBOCs affiliated with a single healthcare system within the VA Midwest Health Care Network will also be presented. Cross-cutting themes identified from these cases offer qualitative context related to the experiences of primary care staff within both rural and urban CBOCs working to implement the PACT model. Intended audience: all clinicians and researchers interested in primary care and rural health.
