Description: PACT Intensive Management is a demonstration funded by VA Office of Primary Care Services to innovate and evaluate care management interventions for high-risk Veterans at five demonstration sites. We will present the 12-month outcomes of this initiative, including costs, outpatient and inpatient utilization, patient satisfaction, and provider satisfaction. Intended audience: primary care clinical staff, primary care leaders, facility leaders.
