A Primary Care PACT to Improve the Health Care of Veterans with Serious Mental Illness: SMI PACT

Description: People with serious mental illness (SMI) have rates of premature mortality three times the general population, mostly due to “natural causes” such as cardiovascular disease and cancer. VA HSRD/QUERI supported the SMI PACT to study the implementation and effectiveness of a specialized PACT team to improve the healthcare of patients with SMI. The SMI PACT model includes structured proactive nurse care management and integrated psychiatric treatment using collaborative care. In a controlled trial, SMI PACT was feasible, well received by patients, and resulted in improvement in treatment and outcomes. Strategies for implementation of this care model will be discussed. Intended Audience: Researchers, Clinicians, Managers, and Policymakers.
