Description: Assessing Circumstances and Offering Resources for Needs (ACORN) is a Department of Veterans Affairs (VA) healthcare system clinical intervention that aims to identify and address social risks and social needs. Through a collaborative effort between the Office of Health Equity and the Office of Rural Health, two projects that are currently underway to understand barriers and facilitators to ACORN implementation in rural settings and assess prevalence and correlates of social risks and receipt of resources/referrals among rural and urban Veterans at the Leave No Veteran Behind: Advancing Veteran Health and Wellbeing Through Action March Cyberseminar session.
