Patient Aligned Care Teams (PACT) Demonstration Labs

The Why and How of Patient-Centered Care: Learning from OEF/OIF Veterans

by Ray Facundo, MSW ; Gala True, PhD
Seminar date: 6/18/2014

Description: The VA’s reorganization of primary and specialty care around the Patient Aligned Care Team (PACT) model emphasizes patient-centered care. To provide patient-centered care, we must understand Veterans’ experiences and perspectives on receiving care through VA. In this presentation, we report on findings from a community-based participatory research project with OEF/OIF Veterans to address several key questions: 1. Why is it important to provide patient-centered post-deployment care?; 2. What are the consequences of lack of patient-centeredness?; and 3. What steps can VA take to improve delivery of patient-centered care to OEF/OIF Veterans? Intended audience includes: members of Patient Aligned Care Teams in Primary and Specialty Care Clinics, post-deployment mental health providers, HSR&D Researchers, and VA administrators.
