Ordin DL (VA Central Office-Office of Quality and Performance)
Parlier R (VA Central Office-Advanced Clinic Access Initiative)
Murawsky J (Hines VAMC-Medicine Service)
Kochevar LK (Minneapolis VAMC-Center for Chronic Disease Outcomes Research)

Objectives:
The mission of the Colorectal Cancer Quality Enhancement Research Initiative (CRC-QUERI) is to promote the implementation of research discoveries and evidence-based care to reduce CRC incidence, late detection, suffering, and mortality among all veterans. Noting the need to reduce the time from positive screening test to diagnosis and increase the use of guideline concordant care, the CRC-QUERI, VA Office of Quality and Performance, and VA Advanced Clinic Access (ACA) Initiative have partnered to conduct the Colorectal Cancer Care Collaborative (C4). C4 includes one team from each VISN. Successful strategies developed by the teams will be disseminated VA wide. C4 is the first time that the primary partners have worked together to implement a quality improvement collaborative. As a result, this effort represents a melding of perspectives and skills with the common goal of clinical improvement. Our objective for this workshop is to discuss the collaborative as a model for organization and implementation of quality improvement partnerships among VA clinicians, managers, and researchers.

Methods:
The workshop will be comprised of a structured panel discussion with significant opportunities for discussion throughout. We will highlight opportunities and challenges that have been encountered in the areas of (1) combining perspectives and cultures of the partners, (2) assembling and utilizing data, and (3) helping teams maximize success. Long-term goals for the collaborative will also be discussed. George Jackson, program manager for the cancer treatment phase of the collaborative, will present an overview of the project. Dede Ordin, Director of Quality Improvement in the Office of Quality and Performance, will discuss VACO’s role in developing the collaborative and bringing together partners. Renee Parlier, ACA Clinical Program Manager, and Jeff Murawsky, C4 clinical champion and coach coordinator, will discuss the principles of ACA and how they are utilized in C4. Laura Kochever, CRC-QUERI Research Coordinator, will discuss data extraction and interpretation. Dawn Provenzale, CRC-QUERI Co-Clinical Coordinator, will discuss challenges in data abstraction and plans for developing a potential set of cancer quality measures.

Results:
Researchers, clinicians, and managers interested in (1) implementation research within the framework of quality improvement and/or (2) developing such initiatives.

Implications:
Basic knowledge of performance improvement.
