Abstract:

This brief educational tool informs community organizations and providers about options for electronic access to VA Health Record information for care coordination. It provides resources to learn more information and also introduces My HealtheVet an option for receiving VA health record information directly from patients. The target audience for this outreach education is community organizations and providers receiving Veteran records through release of information for treatment.
