Description: The Coordination Toolkit and Coaching (CTAC) project is a quality improvement initiative that aimed to improve patients’ experience of care coordination within VA primary care and between VA primary care and other outpatient settings. Presenters will provide preliminary findings from both qualitative and quantitative analyses of the project. Intended Audience: This presentation will be of interest to primary care providers and staff, health services researchers who focus on implementation science, and improvement facilitators.
