Description: The Patient Care Assessment System (PCAS) 2.0 is a nationally-available web-based application to support PACT teams in care management and coordination of their patient panels, with special emphasis on population health and high risk patient identification. In this presentation, learn about the PCAS 2.0 sub-population filters, risk identifiers, care planning tasks and reminders, and a wide variety of VA and non-VA care. In addition, CAN is a foundational component of PCAS, giving providers a quick way to identify some of their highest risk patients. Learn more about the 2.0 CAN scores and how they are being used in VA care. Intended audience: VA leadership, clinical staff, and researchers.
