Delivering Home-Based Cardiac Rehabilitation to Veterans

Description: Referral to cardiac rehabilitation is one of nine performance measures for patients with ischemic heart disease, but only 10% of eligible Veterans receive this guideline-recommended therapy. Dr. Whooley will describe VHA’s implementation efforts to improve delivery of cardiac rehabilitation and increase participation among Veterans. Intended audience: VHA health care providers.
