Description: This presentation will describe a process used to understand the practices that VHA PACT clinicians use to engage patients. Using a Delphi method, this comprehensive list of patient engagement practices was prioritized to create a web-based toolkit for dissemination to PACT providers with the goal of improving patient engagement. This Cyberseminar is intended for a general audience with interest in primary care and team-based care delivery.
