Description: The Patient Care Assessment System (PCAS) is a nationally-available web-based application to support PACT teams in team-based care planning, patient-centered care management, and overall coordination of their patient panels, with special emphasis on patient cohorts and high risk patient identification. In this presentation, learn about new functionality in the third release of PCAS, including dynamic care management, care planning tasks and reminders, and a wide variety of VA and non-VA care enhancements. In addition, CAN is a foundational component of PCAS, giving providers a quick way to identify some of their highest risk patients. Learn more about the CAN scores, including how they are being used in VA care and also in research. Intended audience: VA leadership, clinical staff, and researchers.
