Clinical Quality and the Patient-centered Medical Home: Data from the National PACT Evaluation

Description: Despite widespread adoption of the patient-centered medical home (PCMH) model, it is unknown which components are most related to clinical quality. Dr. Nelson will present data on the association between elements of the PCMH model and clinical quality in the Veterans Health Administration’s (VHA) national PCMH initiative, the Patient Aligned Care Team (PACT) program, recently published in JAMA Internal Medicine. Intended audience: Health services researchers, primary care physicians and providers, and others interested in the patient centered medical home model for primary care practice.
