Description: Communication failures between providers are a threat to patient safety. The ED-PACT Tool facilitates rapid, reliable and brief communication between the Emergency Department (ED) and PACT teams for Veterans with urgent or specific primary care follow-up care needs after an ED visit. This presentation will describe the ED-PACT Tool, as well as its formative evaluation and implementation at VA Greater Los Angeles Healthcare System. Intended audience: VA Primary Care leaders and providers, PACT staff members, Emergency Department leaders, VA Facility leaders, and VA Informatics leads.
