Description: This webinar will provide an overview of the clinical characteristics and needs of Veterans who are at high-risk for hospitalization. Presenters will focus on two partnered research studies: 1) latent class analyses that reveal subgroups of high-risk patients with distinct clinical risk factors and utilization patterns, that may inform and facilitate care management for high-risk patients, and 2) a randomized evaluation of the effects of an intensive primary care program on high-risk Veterans’ utilization patterns, cost, experience, continuity and access, and end-of-life care. Presenters will also discuss an ongoing partnership with Primary Care Operations and the PACT Evaluation Coordinating Center to examine the sociobehavioral characteristics and clinical trajectories of high-risk Veterans. Intended audience: Clinicians and researchers interested in improving clinical care and experiences for high-risk Veterans.
