Preventing Fracture(d Care): Lessons Learned from the Implementation of a Bone Health Care Teleheath PACT

Description: In this Cyberseminar presenters will share lessons learned from the implementation and evaluation of a specialty care telehealth PACT serving rural Veterans at risk of fracture. The Rural Bone Health Team delivery model design and implementation will be described, followed by a report on the experience of primary care providers co-managing care with the specialty care PACT. We also will share the results of a qualitative study of barriers to sustaining bone health delivery models enterprise wide.

Intended Audience: Health services researchers, implementation scientists, and VA healthcare providers who deliver bone health care.
