Primary Care Teamlet Formation and Evolution: Implications for Women's Health

Description: The implementation of the Patient Aligned Care Team (PACT) model has drastically altered VA primary care team roles and relationships. This cyberseminar will highlight the facilitators and barriers to the implementation of new primary care team member roles and incorporating key PACT changes, including structured teamlet communication, continuity improvement, team coordination and role clarity, using small tests of change for practice improvement. The implications of the early teamlet experiences of PACT implementation for important transformations occurring in VA women's health will be emphasized.
