Implementation of the Hospital to Home (H2H) Heart failure Initiative

Description: This cyberseminar will describe the Hospital to Home (H2H) Initiative of the Institute for Health Care Improvement (IHI) and the American College of Cardiology. This initiative was designed to reduce readmission rates by 20% for patients with heart failure and acute myocardial infarction by improving the transition of care. We will describe CHF QUERI’s methods of engaging VA facilities in H2H, and show our impact on VA facility enrollment in H2H and project initiation. In addition, we will provide preliminary data on the association between H2H enrollment, project initiation, process of care and readmission rates.
