Description: Persistently poor diabetes mellitus (PPDM) is a significant health concern, affecting 10-15% of diabetic patients and contributing disproportionately to morbidity, mortality, and expenditure within healthcare systems. A comprehensive telehealth intervention (PRACTICE-DM) improved outcomes in persistently poorly controlled type 2 diabetes compared with a simpler telehealth intervention; because it was explicitly designed for feasible use in clinical practice, translational efforts are possible in systems that need to improve diabetes control in which the requisite infrastructure is available. The aim of this project is to assess implementation determinants of PRACTICE-DM for patients with PPDM. This presentation will summarize the findings and provide insights into implementation strategies for deploying PRACTICE-DM and highlight opportunities to adapt or refine the approach to support uptake in diverse, real-world practice settings.
