Description: This cyberseminar focuses on evaluation of VISN-level training programs to assist PACT teams with implementation of the patient-centered medical home model of care. Over the past year, VISN 4 implemented an innovative Virtual Collaborative to facilitate PACT implementation and spread across the network’s 56 primary care clinics. Anneliese Butler, MSW (VISN4 Demo Lab) will present key findings from a survey of learner experiences conducted as part of a larger formative evaluation of the Virtual Collaborative, highlighting implications for enhancing the effectiveness of future training/spread efforts in VISN4 and VHA more broadly. VISN 22 supported an innovation project called TEX (Team Communication Exercise) to assist PACT teamlets with effective communication. Carole Warde, MD (VISN22 Demo Lab) will summarize the development, implementation and formative evaluation of this practice-based coaching program, discussing lessons learned and recommendations for other programs interested in improving teamwork.
