Patient Aligned Care Teams (PACT) Demonstration Labs
Dual use of VA and non-VA Services by Veterans in PACT
Mary Charlton, PhD
Gary Rosenthal, MD
Carolyn Turvey, PhD
Seminar date: 5/15/2013
Description: This cyberseminar focuses on the issue of dual use of VA and non-VA services among Veterans and how it presents unique challenges for PACTs working to provide comprehensive, coordinated care for these patients. Gary Rosenthal, MD (VISN 23 Demo Lab) will present findings on the prevalence and utilization patterns of dual users and discuss why this is a concern for primary care teams implementing the PACT model. Mary Charlton, PhD (VRHRC–CR) will present findings on the perceptions of VA and Non-VA providers, as well as veterans, regarding care coordination and health information exchange for Veterans who receive care from both systems. Examples of innovative efforts to facilitate information exchange from various perspectives will be provided, including: the Co-Management Toolkit developed by the Veterans Rural Resource Center-Central Region; work conducted by Carolyn Turvey, PhD involving a novel use of the “Blue Button” feature through MyHealtheVet; and examples of VA sites piloting models for electronic health information exchange between VA and non-VA facilities.
