Engaging Caregivers in the Care of Veterans with Dementia and Diabetes

Description: We will discuss preliminary results from recent research studies that uncover the ways that veteran's family members get involved with veterans' diabetes self-management and medical care. Specifically we will focus on how family members interact with the PACT team, help patients prepare for and communicate during PACT appointments, and help patients enact clinical recommendations and navigate the VA health system. At the end we will discuss ways that PACT teamlets might engage these family members to better help patients who are struggling with diabetes management. Additionally, this talk will present preliminary findings from a randomized trial of a telephone-administered care management program for caregivers of Veterans with dementia. Issues surrounding program feasibility and whether, relative to usual care (UC), the program was associated with improved caregiver and patient outcomes will be discussed. The target audience is PACT clinical staff, primary care administrators, caregiving coordinators, PACT and chronic disease management researchers.
