The Patient Care Assessment System (PCAS): A New Tool to Support PACT and High Risk Patient Management

Description: This cyberseminar will provide an introduction to the Patient Care Assessment System (PCAS), a new tool now available to PACT members. The foundational philosophy of PCAS - and the focus of this first release - is to help PACT providers, and in particular PACT care managers, understand the universe of risk characteristics for their patient panels and be able to target the most appropriate care at the right time, to the patients who need it most. Risk information provided in the first release of PCAS includes the Care Assessment Needs (CAN) scores statistical models and other clinical and cost risk information. The intended audience is PACT members and leadership, as well as other VHA leadership or primary care staff.
