Assessment of PACT Structure and Organization using Social Network Analysis

Description: The PACT model guidelines suggest that Veterans' care should be coordinated by teams consisting of a Provider, RN Care Manager, Clinical Associate, and Clerical Associate. We use social network analysis procedures to graphically portray and measure differences in PACT structure that occur at the health care system level. We link these measures to performance data to assess how differences in the way PACT teams are organized relate to access, continuity of care, and coordination of care.
The target audience includes primary care leadership, clinicians, and researchers interested how variation in PACT structure relates to care outcomes.
