Abstract:

Primary Care - Mental Health Integration (PC-MHI) embeds mental health specialists in primary care and promotes nurse care management of depression through evidence-based collaborative care models. In order to improve Veteran access to mental health services, PC-MHI and PACT work together to treat primary care patients with low-to-moderate complexity mental health conditions within the medical home. This presentation describes increasing primary care clinic engagement in PC-MHI over time and its effect on healthcare utilization and costs for Veterans in VISN 22. Intended audience: VA primary care leaders, providers, and other PACT teamlet staff; VA mental health leaders, providers, and PC-MHI staff; VA facility and VISN leaders.
