Abstract:

Workshop Objectives: As part of the VA transformational initiative to establish new models of care, VHA recently established the Patient Aligned Care Teams (PACT) initiative to create patient-centered medical homes as the basis of primary care throughout VHA. VHA primary care leaders established PACT Demonstration Laboratories to evaluate and enhance the system-wide transformation of primary care. Five Demonstration Laboratories are designing and evaluating PACT-related innovations for subsequent spread and conducting regional evaluations of different facets of PACT roll-out. A Demonstration Lab Coordinating Center is tasked with coordination of Lab activities and national evaluation of PACT deployment. This Workshop is designed to provide HSRandD researchers with an overview of the plans and activities of the five funded PACT Demonstration Labs and the Coordinating Center. Workshop leaders will discuss the research-clinical partnerships that underlie Demonstration Lab and Coordinating Center activities, as well as opportunities for participation in PACT innovations and evaluation. Activities: This session will include presentations by PCS leadership involved in designing the PACT Demonstration Lab program and by Demonstration Lab Directors responsible for PACT clinical innovations and evaluation. Speakers will include Richard Stark, MD, Director, Primary Care Clinic Operations; Stephan Fihn, MD, Director of the PACT Demonstration Laboratory Coordinating Center; and Directors of the five PACT Demonstration Laboratories: Judith Long, MD (VISN 4); Eve Kerr, MD (VISN 11); David Hickam, MD (VISN 20); Lisa Rubenstein, MD, MSPH (VISN 22); and Gary Rosenthal, MD (VISN 23). Given the scope of VHA's national PACT roll-out, ample time will be scheduled for discussion and feedback from VA clinicians and researchers. Target Audience: The target audience for this workshop is HSRandD researchers interested in learning about the PACT Demonstration Laboratories and/or participating in research related to the PACT roll-out. Assumed Audience Familiarity with Topic: The workshop assumes limited knowledge of PACT on the part of HSRandD researchers who are not involved in clinical activities; clinician-researchers who are participating in the PACT clinical transformation may be familiar with PACT activities at the local hospital or clinic level.
