Designing Systems of Care: Developing an Interdisciplinary Weight Management Program. Invited oral presentation, VISN 21 Let’s MOVE! Mini-Residency, Berkeley, CA

Frayne SM. Designing Systems of Care: Developing an Interdisciplinary Weight Management Program. Invited oral presentation, VISN 21 Let’s MOVE! Mini-Residency, Berkeley, CA. 2012 Jan 18.

Abstract:

"Let's MOVE! The Weight is Over" This innovation provides Veterans with education and resources to help control their weight, including the creation of a multidisciplinary weight loss clinic in each facility, as well as videoconferencing to allow patients to attend appointments remotely. Staff are also provided with intensive training to help Veterans meet their weight goals. Helping Veterans control their weight will lead to fewer weight-related health complications, improving Veterans' overall health and quality of life. This project will be field-tested in facilities within VISN 21 (Sierra Pacific Network)
