Abstract:

This symposium will address the development, implementation, and operation of a state-wide collaborative care program that delivers telephone-based care management, monitoring, and other support for mental health treatment of older Pennsylvanians who are covered by a state-sponsored prescription drug plan, and whose primary care providers have prescribed a psychotropic medication under that plan.
