Description: We will describe the: • Development of the Community Hospital Transitions Program; a transitions of care program for Veterans who are hospitalized in the community and return to VA primary care. • Pre-implementation, early implementation and implementation activities we conducted at the two VA healthcare sites that adopted the Community Hospital Transitions Program. Intended audience: VA researchers, program staff, clinicians, and fellows interested in the development, implementation and dissemination of a quality improvement intervention or program.
