Description: This presentation will include analyses looking at responses from the Survey of Health Care Experiences (SHEP), PCMH item set and hospitalization for ambulatory care sensitive conditions. Five specific ways to access primary care have been highlighted in the SHEP-PCMH and used for metrics of quality and performance within VHA. We will review the results of our analyses looking at if better experiences are associated with a reduction in hospitalization. Intended Audience: health services researchers, primary care administrators, and clinicians.
