Description: Dr. Cynthia Perry and Laura Damschroder will present their work related to the AHRQ-funded ESCALATES National Evaluation of seven large cooperatives who used implementation strategies to encourage use of evidence-based practices to reduce cardiovascular risk among primary care patients.
