Description: Abstract:Practical tools such as implementation field guides, playbooks, and guidebooks can assist in implementation of a project. The presenters will describe their experience in creating and outlining process focused documentation for VA primary care clinics implementing a population health management hypertension project. This presentation will focus on key considerations and lessons learned in the development of an implementation field guide for improvement project stakeholders.

Target audience: Project coordinators, implementation specialists, implementation scientists, and others involved in planning and conducting program implementation.
