Description: Minority Veterans often have unique barriers to overcome around diabetes self-management. The May 2022 Focus on Health Equity and Action Cyberseminar highlights a quality improvement (QI) project used the Primary Care Equity Dashboard (PCED) and focus groups to identify unique barriers and recommendations to improve diabetes self-management in non-Hispanic Black and Hispanic/Latino/a/x Veterans living with poorly controlled diabetes (i.e., HA1c >9). The results of this project were integrated within a social determinants of health model and were used to inform a multifaceted Whole Health group intervention using functional medicine to help reduce racial and ethnic disparities in diabetes control among Veterans.

Researchers, clinicians, care coordinators, policy makers, educators, data analysts, Veteran stakeholders and representatives of vulnerable Veteran groups who are interested in understanding and/or promoting equitable health among all Veterans.
