Promoting Healthcare Equity in Veterans with a Whole Health Approach: Focus on Hemoglobin A1c Control in Patients with Diabetes

Description: Minority Veterans often have unique barriers to overcome around diabetes self-management. The May 2022 Focus on Health Equity and Action Cyberseminar highlights a quality improvement (QI) project used the Primary Care Equity Dashboard (PCED) and focus groups to identify unique barriers and recommendations to improve diabetes self-management in non-Hispanic Black and Hispanic/Latino/a/x Veterans living with poorly controlled diabetes (i.e., HA1c >9). The results of this project were integrated within a social determinants of health model and were used to inform a multifaceted Whole Health group intervention using functional medicine to help reduce racial and ethnic disparities in diabetes control among Veterans. Target audience:
Researchers, clinicians, care coordinators, policy makers, educators, data analysts, Veteran stakeholders and representatives of vulnerable Veteran groups who are interested in understanding and/or promoting equitable health among all Veterans.
