Description: This session will describe the strategies, approaches, and results for disseminating, adapting, and measuring uptake of several patient-facing tools. These tools are patient-facing materials that aim to improve care coordination in primary care by helping Veteran patients understand how and when to access different kinds of care in the VA and community. We will discuss how we used human-centered design approaches to develop and adapt these tools concurrently with our dissemination efforts. Audience: The intended audience is frontline clinicians who may be interested in using these or other patient-facing materials to improve care coordination, and researchers or developers of patient-facing materials interested in using human-centered design approaches.
