Grim Tales: Three Stories of VA Documentation Failures

Description: In this Cyberseminar presenters will discuss lessons learned from a recently concluded research project on post procedure adverse advent detection. They will examine three use cases that they discovered where both VA structured and unstructured data did not contain what was expected. These cases will be offered as cautionary tales against blind trust of data. In each case the data was compared against human chart review as a gold standard.
