Improving Glaucoma Medication Adherence

BACKGROUND/RATIONALE:
Glaucoma is the leading cause of irreversible blindness worldwide and disproportionately affects the elderly and African-Americans in both prevalence and severity. The number of Americans with glaucoma is expected to increase by 50% in the next 15 years. Multiple studies have shown that with effective medical treatment, much glaucomatous vision loss can be prevented through reduction in intraocular pressure. Despite this, glaucoma medications often are not taken as prescribed.

OBJECTIVE(S):
The overall objective of the projects included in this career development award is to reduce vision loss in veterans with glaucoma by identifying and reducing barriers to glaucoma medication adherence.

METHODS:
The first study of this program is an observational study comparing different methods for quantifying glaucoma medication adherence: electronic monitors, self-report, pharmacy refill records, and direct observation of drop instillation technique. These methods of measuring adherence will be compared to each other and to clinically relevant outcomes including intraocular pressure and visual field deterioration. The second study involves the qualitative assessment and refinement of a prototype intervention to improve medication adherence. Finally, the refined intervention will be pilot tested in patients with glaucoma.

IMPACT:
The successful completion of these projects led to the study design proposed for a randomized controlled trial described in a recent IIR application. If successful, the intervention proposed will lead to less preventable vision loss in veterans. Blindness from glaucoma leads to worse quality of life and loss of independence. By reducing glaucoma-related blindness we hope to maintain quality of life and independent living for veterans affected by glaucoma.
