Developing a Couples Coping Intervention for Veterans with Heart Failure

BACKGROUND/RATIONALE:
Chronic heart failure (CHF) is the number one discharge diagnosis among Veterans. Partners (spouses/significant others) are integral to self-management but are seldom targeted in self-management interventions. Engaging couples and other patient-caregiver dyads may be effective in improving patient outcomes provided this does not lead to additional burden.

OBJECTIVE(S):
To develop and test a couples' based coping and self-management intervention for Veterans with CHF and their spousal caregivers.

METHODS:
The research proposed in the CDA involves developing a treatment manual after obtaining input from providers, Veterans with CHF, and their partners; pilot test the acceptability and feasibility on a sample of 15 Veterans with heart failure and their partners; refine the intervention based on the results of the pilot study; and lay the foundation for an RCT to determine its efficacy in improving quality of life, couples' coping, and self-management among Veterans with heart failure and their partners.

IMPACT:
CHF is a deadly and expensive disease. Self-management is at the core of improving outcomes but adherence to self-management recommendations remains poor. The CDA research addresses the HSR&D Priority area of "long term care and caregiving." Beyond the specific focus of the CDA, the CDA has successfully allowed the PI to expand and develop the program of research to address the needs of chronic illnesses including heart failure, depression, diabetes, and complex chronic conditions.
