This HSR&D CDA award will develop a research and training agenda that aims to advance the science of Veterans' participant-directed (PD) home and community-based service (HCBS) utilization and outcomes research by establishing a program of research that will evaluate access to these programs and their effectiveness. The project will 1) determine which eligible Veterans being served by which Medical Centers enroll in VA's PD cash benefit programs, 2) explore reasons for variation in enrollment in VD-HCBS, A&A and Housebound benefits across VAMCs, 3) describe Veterans' experiences with the VA's PD cash benefit programs, 4) establish the effectiveness of the VA's PD cash benefit programs in controlling healthcare costs by comparing Veterans' healthcare utilization to that of Veterans receiving traditional LTSS, and 5) obtain complementary training in program evaluation and implementation research utilizing comparative effectiveness, primary data collection, and qualitative research methodologies. Dr. Thomas' current research broadly encompasses two areas: the organization and delivery of long-term care and measurement in long-term care. She has more than 9 years of long-term care research experience and is an expert in using large administrative datasets (e.g. OSCAR, MDS, OASIS, Medicare Claims, and Medicaid Cost Reports) to ask and answer policy relevant questions.
