Optimizing Care of Atrial Fibrillation an Atrial Flutter

BACKGROUND/RATIONALE:
The overarching goal of the work is to optimize care of atrial fibrillation and atrial flutter (AF, collectively) in the Veterans Health Administration (VHA), and to develop performance and quality measures for both of stroke prevention and arrhythmia management.

AF is exceedingly common in veterans. In veterans, the prevalence of AF among 29,587 outpatients receiving electrocardiograms in the VA Palo Alto Health Care System (VAPAHCS) was 8.3%. Due to rising rates of obesity and hypertension in young adults, the age-adjusted incidence of AF increasing and is therefore expected to affect more Veterans at younger ages.

OBJECTIVE(S):
Objective 1: To evaluate the quality of anticoagulation care in the VHA for stroke prevention in patients with AF

Objective 2: To evaluate the quality of arrhythmia management care in Veterans with AF

Objective 3: To pilot a screening program for AF in patients at high-risk for AF

METHODS:
For Objectives 1 and 2, I will perform two retrospective studies using linked VA and Medicare administrative data. For Objective 1, I will use an existing database to 1) characterize processes of care for stroke prevention for AF in the VHA; 2) determine the relationship of patient, provider, and facility characteristics to processes of stroke prevention care; 3) determine clinical outcomes associated with processes of stroke prevention care. For Objective 2, I will link this dataset to additional data sources to 1) characterize the use of different AF management strategies in the VHA; 2) identify facility-, provider-, and patient-level predictors of strategy selection; 3) determine clinical outcomes associated with each treatment strategy. For Objective 3, I will pilot a screening program for AF in patients at high risk of having AF. My current work on Objectives 1 and 2 has indicated that the CHADS2 score, used to estimate stroke risk in patients diagnosed with AF, has similar test discrimination in patients without known AF. Extending outpatient ambulatory screening to high-risk patients may permit detection of undiagnosed AF and therefore inform stroke prevention.

IMPACT:
Atrial fibrillation and flutter (collectively, AF) are conditions of very high prevalence among Veterans. Identification of patient-, provider-, and facility-level factors associated with quality of AF care or outcomes across the entire VHA would be the first step to improve care system-wide. These data can help stimulate the creation of VA Office Quality and Performance, and also national and international society guidelines for AF management. Moreover, our pilot screening study has demonstrated the feasibility of screening for silent AF among high risk patients, and the high yield of atrial arrhythmias found in this study could indicate risk of developing of AF or could represent an independent stroke risk factor. Together, these projects have the potential to reduce the overall public health burden of stroke and decrease morbidity, mortality, hospitalizations, and expenditures related to management of AF.
